Louisiana Long-Term Personal Care Services (LT-PCS)
Long-Term Personal Care Services (LT-PCS) is a Louisiana Medicaid state plan service that helps older adults and adults with disabilities with daily activities at home. It has no waitlist, which makes it a key option while people wait for the Community Choices Waiver. This page is part of our Louisiana Long-Term Care Medicaid guide.

LT-PCS in brief
To sum up, Louisiana Department of Health, Office of Aging and Adult Services (OAAS) oversees LT-PCS. Who it is for: 65 and older, or 21 to 64 with a disability. Care need required: Meets nursing facility level of care and needs help with daily activities. As for waiting: No. The sections below repeat these points with more detail, and our state guide covers the Louisiana rules that apply to every long-term care program. If LT-PCS is not the right fit, the related pages at the end point to other Louisiana programs and general guides.
What the program is
OAAS manages LT-PCS. An assessment sets the number of hours, and a provider agency (or a self-directed arrangement) delivers the care. Participants in managed care may access LT-PCS through their plan.
Because LT-PCS is a state plan service, it works differently from a waiver. State plan benefits generally cannot have enrollment caps, so a person who has full Medicaid and an assessed need for hands-on help can get the service without waiting for a slot. It is narrower than a waiver, though: the focus is help with daily activities, not the broader range of waiver services. See In-home care.
Who qualifies in 2026
Eligibility for LT-PCS comes down to these points. The person:
- Is 65 or older, or 21 to 64 with a disability
- Has Louisiana Medicaid
- Meets a nursing facility level of care and needs help with activities of daily living
- Lives in Louisiana and is a U.S. citizen or qualified non-citizen
The money test. The person must have Louisiana Medicaid. Louisiana’s rules are in our state guide. Figures shown above are the 2026 Louisiana long-term care limits from our state guide. the Louisiana Department of Health’s countable assets do not usually include the home (within the equity limit, if the applicant or a spouse lives there or intends to return), one car, personal belongings and a small burial fund. If only one spouse needs LT-PCS, spousal impoverishment rules protect assets and income for the other. See Spousal impoverishment rules.
What the care test means in practice. the Louisiana Department of Health’s assessment covers daily activities, from bathing and dressing to toileting and medications, plus memory and safety. For LT-PCS, an assessor, usually a nurse or social worker, visits the person at home, in the hospital or in the facility. Before a LT-PCS assessment, it helps to line up a family member who can describe a typical bad day, and to have a list of diagnoses and medications ready. See Nursing home level of care and Activities of daily living.
What services it can pay for
What a participant receives under LT-PCS follows the care plan. Possible services include:
- Help with bathing, dressing, grooming and eating
- Help with toileting and transfers
- Meal preparation
- Housekeeping and laundry tied to care
- Shopping and medication reminders
No program pays for everything. LT-PCS services are based on assessed need, and a family caregiver who is already helping is usually counted when the plan is written. When needs grow, request an updated assessment.
How to apply
Getting LT-PCS means passing a money test and a care test. Step by step:
- Make sure the person has Louisiana Medicaid.
- Request LT-PCS through the Louisiana Department of Health’s long-term care access line.
- Complete the assessment.
- Choose a provider agency or self-direction.
Our documents checklist and Applying for a waiver guide list the paperwork.
Waiting lists
LT-PCS has no waitlist.
General advice is in Waiver waiting lists.
How it relates to nursing home Medicaid
LT-PCS uses the nursing facility level of care, so recipients could also qualify for a nursing home. It lets them stay home, often while waiting for the CCW.
Rules shared with nursing home Medicaid. Most money rules carry over from nursing home Medicaid. The five-year look-back still matters: a transfer for less than fair value can delay payment for long-term care services. the Louisiana Department of Health can also use estate recovery to seek repayment for long-term care services received after 55. Under LT-PCS, income above an allowance may go toward the cost of care each month.
Income in a nursing home. In 2026, Louisiana lets a nursing home resident keep $45 a month for personal needs; the rest of their income mostly pays toward care. Details are in our state guide.
Tips for families
- Start early. If there is a waiting list or a slow assessment, the clock usually starts when you ask, not when the crisis comes.
- Document everything, from forms and letters to who you spoke with at the Louisiana Department of Health and when.
- Watch the deadlines on notices. A denial or cut can be appealed through a fair hearing, and a timely request can keep services going meanwhile.
- Tell the Louisiana Department of Health about changes in money, address or household as they happen.
- Expect yearly reviews of both Medicaid eligibility and the care assessment. See Renewals.
- Protect a spouse at home. Spousal protections must be claimed; a certified Medicaid planner or legal aid office can explain them for your family.
Common questions
Can I get LT-PCS and CCW together? CCW includes personal assistance, so the two are coordinated to avoid paying twice for the same help.
Is there a cap on hours? Yes, set by the assessment and program rules.
Can family be paid? Self-direction may allow hiring some relatives.
